It's working. That's the problem.
Three patterns show up in almost every SMB, and in plenty of large companies too. None of them is anyone's fault, and all of them feel normal from the inside. Together they're why the foundation never gets built until something breaks.

The foundation was never skipped out of ignorance. It was skipped because it was expensive, invisible and nobody's job.
Abundance makes it cheap. Agents make it visible: written, checked and logged daily. A held space makes it someone's job without taking anyone's place. Then the superstar can take a holiday, "it's working" becomes "it's held", and the foundation budget turns into evidence instead of discretion.
